National Diabetes Prevention Program Online

Who: To be eligible, participants must be at risk for prediabetes or diagnosed with prediabetes through the following criteria:

  • Be at least 18 years old and
  • Be overweight (Body Mass Index ≥25; ≥23 if Asian) and
  • Not be pregnant
  • Have no previous diagnosis of type 1 or type 2 diabetes and
  • Have a blood test result in the prediabetes range within the past year:
    • Hemoglobin A1C: 5.7–6.4% or
    • Fasting plasma glucose: 100–125 mg/dL or
    • Two-hour plasma glucose (after a 75 gm glucose load): 140–199 mg/dL or
    • Have a previous clinical diagnosis of gestational diabetes

Participants with a diagnosis of Type 1 or Type 2 diabetes are not eligible for this program.

Could you have prediabetes? If you answer yes to a few of the following questions, you may qualify for the National Diabetes Prevention Program.

  1. Are you 45 years of age or older?
  2. Are you overweight?
  3. Do you have a mother, father, sister, or brother with diabetes?
  4. Is your family background African American, Hispanic/Latino, American Indian, Asian American, or Pacific Islander?
  5. Have you ever been diagnosed with high blood pressure?
  6. Did you have diabetes while pregnant (gestational diabetes), or give birth to a baby weighing 9 pounds or more?
  7. Are you physically active less than three times a week?

What: The National Diabetes Prevention Program is designed to bring to communities’ an evidence-based lifestyle change program for preventing type 2 diabetes. It is based on research led by the National Institutes of Health and supported by the Centers for Disease Control and Prevention. Coaches will provide support, encouragement, and information to participants and work through challenges that may arise providing an environment that encourages self-management for the participant’s health and wellbeing.

Why: People with prediabetes are more likely to develop heart disease and stroke. The lifestyle change program proves that making modest behavior changes, such as improving food choices and increasing physical activity to at least 150 minutes per week, results in participants losing 5% to 7% of their body weight. These lifestyle changes reduced the risk of developing type 2 diabetes by 58% in people at high risk for diabetes.

When: An information session will be provided on Tuesday, June 16 2020 from 12:30 p.m. - 1:30 p.m. . Sessions will start Tuesday, June 23, 2020 and repeat weekly for 16 weeks and then monthly for 6 months. As a participant, you will work with a certified lifestyle coach in a group setting learning to adopt healthy lifestyles that include healthy eating and incorporating physical activity and sharing information to assist you with your goal to decrease and prevent your risk of developing type 2 diabetes.
