Chronic Disease PATH Telephone Toolkit
A Self-Management Resource Center Program

Thursdays from 1-2 p.m. ET
Feb. 27 – April 10, 2025

Feeling tired, overwhelmed, or isolated because of your chronic disease? The Chronic Disease PATH Telephone Toolkit program gives you the tools to feel better. You will learn how to:

  • Deal with frustration, fatigue, pain, and difficult emotions
  • Be active for better health
  • Make healthier food choices
  • Talk with family, friends, and healthcare providers about your health

Program benefits may include:

  • Increased physical activity which can lead to better energy levels
  • Improved ways to talk to healthcare providers and family
  • Better quality of life

This workshop is free for Michigan residents. Family members, friends, and caregivers are encouraged to attend with you.

 WORKSHOP:

The workshop meets via telephone for 1 hour, once a week for 6 weeks. Please let your facilitators know if you are going to be absent.
