* Number of years 4-H member Each scholarship recipient must be or have been an active Berrien County 4-H member for a total of at least five years including his or her senior year in high school or equivalence. Individuals applying for the Horse Leaders Association scholarship must be active and enrolled in a Berrien County 4-H horse and pony project for a minimum of two years.

* Name of your 4-H Club(s) or Group: Please list the names of the Berrien County 4-H Clubs/groups you were/are a member of. Also include years of membership. Example: Sunnyside Clovers - Berrien County - 5 Years

* Post Secondary Education Institution Details: List the following for your postsecondary education institution: - Post Secondary Institution Name - Post Secondary Institution City/State - Your anticipated major/minor, certification, program name, etc.
